Individuals who have a TC 96-347 form signed by their physician may bring the form to the County Clerk’s office to receive a disabled parking placard. Bring in your driver’s license. The first placard is free. A second placard is available for $10. Permanent placards are valid for six years from date of issue. Temporary placards are valid for three months from date of issue.

If your doctor determines that you need a disabled parking license plate, you must be the registered owner of the vehicle to receive one. Proof of current Kentucky liability insurance is required. The Kentucky license plate currently issued to your vehicle must be surrendered.

**DO NOT SIGN YOUR APPLICATION UNTIL YOU ARE AT THE COUNTY CLERK’S OFFICE AS YOUR SIGNATURE MUST BE NOTARIZED.
